Considering healthcare as a constitutional right and the growing demand for services in the face of limited budgets, the study examines how court rulings influence resource allocation and the implementation of local public policies. The approach is qualitative, based on documentary analysis of judicial decisions, budget reports, and institutional documents, complemented by secondary data on the municipality’s socioeconomic context. The research draws on authors who address the multiple dimensions of healthcare judicialization. Notable references include Denzin (2006), who provides the foundation for methodological triangulation; Vergara (2016), who guides the research design; Silva (2022) and Vieira (2020), who examine the budgetary effects and sustainability of Brazil’s Unified Health System (SUS); Sampaio Júnior (2009), who proposes institutional solutions within the Judiciary; and the Conselho Nacional de Justiça (CNJ, 2023), which contributes to the debate on the structural causes of the phenomenon. The findings indicate that judicialization, while ensuring access to treatments and medications, generates significant impacts on financial planning, requiring budget reallocations and adjustments in priority-setting. The data show an increase of more than 1000% in municipal expenditures related to healthcare judicialization during the period analyzed, with a large concentration of spending directed to a single accredited clinic. This result highlights the magnitude of the phenomenon and its disproportionate effects on the local public budget. The study concludes that coordination between public managers, judicial institutions, and healthcare professionals is essential to reduce legal demands and optimize resource use, safeguarding efficiency and equity in healthcare delivery.ptJudicialização da saúdeGestão pública municipalPolíticas públicas
