In Brazil, the National Accreditation Organization (ONA) is one of the main models adopted by healthcare institutions, as it establishes requirements focused on standardization, performance assessment, and the development of an organizational culture oriented toward patient safety. Despite challenges such as costs, time, and institutional engagement, accreditation contributes to increased patient trust, improved institutional reputation, and more efficient healthcare services. Objective: To assess the organizational maturity of a public hospital in Imperatriz, Maranhão, using an instrument based on ONA accreditation requirements, demonstrating the feasibility of initiating institutional reform through the hospital’s quality and risk management sector. Materials and Methods: This is a cross-sectional analytical study conducted at the Municipal Hospital of Imperatriz, Maranhão, using an organizational maturity assessment tool based on the Brazilian Accreditation Manual criteria. Evaluations were carried out at two annual time points throughout 2023 and 2024, covering multiple dimensions related to healthcare quality and organizational performance. The methodology included internal audits every six months, involving a multidisciplinary hospital team, identifying conformities and nonconformities and supporting the development of a strategic plan aimed at restructuring and improving healthcare service quality. Results: An improvement in the hospital’s organizational maturity was observed between 2023 and 2024, highlighting progress in management, quality, and patient safety. Leadership engagement increased, positively influencing healthcare professionals’ perceptions. Patient participation and satisfaction improved, although challenges remain in systematically incorporating feedback into decision-making processes. Risk management and professional training also progressed; however, infrastructure limitations and team integration continued to be critical areas requiring ongoing attention. Conclusion: Hospital accreditation and quality management based on ONA standards are essential to support the organizational transformation of the Municipal Hospital of Imperatriz.
