The prevalence, in Goiás, for follow-up of 33% and 8.2%, respectively. Chronic diseases have a high cost for the SUS, requiring high expenses with medical care, hospitalizations and treatment. The general objective of this study is the actions of the Family Health Strategy for the control of SAH and DM, in the municipality of Iporá. This is a qualitative research. The statistical criteria were applied in this study to reach the proposed goals of health analysis for the control of SAH and DM in the municipality of Iporá. The patient has been applied to occupy the health and health care salary (ESF) with patient and problem with assiduity of health careers (ESF) to patient enrolled in program the control of diseases. The Hypertension and Diabetes Surveillance and Monitoring System (HiperDia) has, since its inception, the intention to disseminate the target public and to accompany patients with STD and DMP in order to minimize the aggravations caused by these diseases. Results: it is noticeable that the new actions in the municipality of Iporá are activated in a satisfactory way for a public health policy. It is possible that the actions involve a promotion, prevention and treatment and also provide the support for the users, which goes beyond what is required of the professionals, favoring the positive results of a program.
